Treatment of Unstable Intertrochanteric Fractures of the Femur by Wayne-County reduction in Elderly patients

Abstract
Purpose: The aim of this study was to evaluated the radiological and clinical results of the treatment of unstable inertrochanteric fractures of the femur by Wayne-County reductio in elderly patients.

Materials and Methods: Forty-eight fractures were treated with a 135-degree angled compression hip screw by valgus reductin. The average patient age was 72.4(65-92) and women numbered thirtynine. All cases displyaed unstable fractures by Jensen classification. The average duration of follow-up for all fractures was fifteen months(12-41).

Results: Six patients died before the minimum one-year follow-up period had elapsed. The neckshaft angle averaged 134 degrees in the final follow-up films. The sliding length of the lag screw was 6.6 §® and the penetrating length of the lag screw into the femoral head was 2.2 §®. The time of bony unoin was 4.2 months and the leg length discrepancy was 4 §®. Loss of fixation, with varus angluation of the fracture, occurred in two patients(4.8%). Migratinor nonunion of the greater trochanter developed in four cases. Among the thirty-five independent community ambulators before injury, thirty-one(88.6%) recovered by the postoperative one year follow-up.

Conclusion: Sliding compression hip screw fixation with Wayne-County reduction is a recommended method in the treatment of unstable intertrochanteric fractures of the femur in elderly patients in which it is difficult to obtain anatomical reduction.
